Description:

Supporting young adults who use AAC as they transition from high school requires an individualized, person-centered approach. This session will outline practical strategies to support continued communication independence, including updating AAC systems (low-tech and speech-generating device components) to meet evolving needs in self-advocacy, independent living, and functional communication. It will also define the role of a designated AAC support person in training communication partners, maintaining system access, and preparing for new environments. Finally, participants will be introduced to two tools, a structured Coaching Plan and a Communication Passport, that can enhance an Individualized Support Plan (ISP) or provide continuity when one is not in place.

Learning Outcomes:

As a result of this activity, participants will be able to:

• As a result of this activity, participants will be able to list one self-advocacy, one independent living, and one functional communication update that an individual's AAC system may need to support their transition to life after high school.

• As a result of this activity, participants will be able to develop a Coaching Plan to meet the needs of an individual who uses AAC in their transition to life after high school.

• As a result of this activity, participants will be able to create a Communication Passport for an individual who uses AAC systems.
