Description:

Inglis has launched a pilot program that combines assistive technology (AT) and home modifications (HM) into a unified service model supporting older adults who wish to age in place. This innovative approach bridges service gaps and provides person-centered solutions that enhance safety, independence, and quality of life. The session will highlight why and how this partnership began, share key outcomes and demographic data, and present case studies illustrating impact.

Learning Outcomes:

As a result of this activity, participants will be able to:

• Attendees will evaluate and apply three strategies for integrating assistive technology and home modifications into a unified, person-centered model that supports aging in place.

• Attendees will analyze and leverage two strategic partnerships and targeted grant funding (e.g., foundations) to design and pilot innovative service delivery approaches.

• Attendees will interpret two case study outcomes from the pilot program to inform future program development and scaling decisions.
